Institutional / Nursing Home Medicaid & Home and Community-Based Services (HCBS) Waivers

These programs provide long-term care either in a nursing facility or through services at home or in the community.

  • Single Applicant:

    • Income Limit: $2,901 per month

    • Asset Limit: $2,000

  • Married Couple (One Spouse Applying):

    • Applicant Income Limit: $2,901 per month

    • Applicant Asset Limit: $2,000

    • Non-Applicant Spouse Asset Allowance: A portion of the couple’s assets up to $157,920

  • Married Couple (Both Spouses Applying):

    • Income Limit: $5,802 per month (combined)

    • Asset Limit: $3,000

 

Note: When only one spouse applies, only the applicant's income is considered. The non-applicant spouse may be entitled to a portion of the applicant's income to prevent impoverishment, known as the Monthly Maintenance Needs Allowance (MMNA). In 2025, the MMNA in Oklahoma is $3,948.

 

 

💡 What Counts as Income?

Medicaid considers most income sources, including:

  • Employment and wages

  • Social Security benefits

  • Pension payments

  • IRA and 401(k) withdrawals

  • Dividends and interest

  • Rental income

 

Exclusions: Certain payments, such as Holocaust restitution, are not counted as income. 

⚖️ Exceed The Income Limit?

Oklahoma is an "income cap" state, meaning that individuals with income exceeding the limit cannot qualify for Medicaid long-term care benefits unless they establish a Qualified Income Trust (QIT), also known as a Miller Trust. This trust allows excess income to be set aside and not counted towards eligibility. A QIT has to be managed correctly and the accumulated income in the trust is eventually paid to the Oklahoma Health Care Authority.
